The information you give on this form is collected by the St John the Baptist Parish 300 Club, which is the data controller for the 300 Club. We use it only to run your membership and the monthly draw: to keep the club register (name, address, entries and payment records), to contact you about your membership and winnings, and to meet our record-keeping obligations under the Gambling Act 2005. We never sell or share your details with anyone else, and we only pass information to the parish bank for setting up your standing order if you join that way. Records are kept for as long as you remain a member, plus the period required for lottery registers after you leave. You can ask at any time to see, correct or delete your details by contacting the parish office or any club organiser.
